SITE REVIEW APPLICATION FORM
The Application for Site Review is to be completed by the applicant and submitted via email in PDF format to sitereview@valpo.us or in person to the Office of the Clerk Treasurer along with the processing fee of $100 payable to the City of Valparaiso.
| • | Click here to fill out Site Review Application Form online. This options allows you to type in the information on the Site Review Application Form online. Type in your information in the colored boxes; hit the tab key to move from field to field. When finished, click the Submit Form button. After clicking the Submit Form button, your new message email form will open with your Site Review Application Form attached as a PDF. The form and any other information that you include in the email will be sent to sitereview@valpo.us. You can then close the form Windows tab. |
| • | Click here to print out the Site Review Application Form. This options allows you to print down the Site Review Application Form to fill out and return in person to the Office of the Clerk Treasurer. |
NOTE: Eleven (11) sets of plans must be submitted ONE (1) WEEK prior to the scheduled date or the presentation will be removed from the schedule.
