CONSTRUCTION FORMS

IMPORTANT: New construction permit applications and permit application for any other projects that involve site work (this includes earth moving, sewer work, and right-of-ways cuts) must also include a Site Permit Application. Site Permit Applications are available at City Hall.

We have provided several forms below for the Builder / Contractor.  These forms have been saved as a PDF file.  You will need to download or have a copy of Adobe Acrobat Reader 4.0 or better on your computer in order to view it.  In order to download the latest version of Adobe Acrobat Reader, please access Adobe's website.

Application for Contractors / Subcontractors / Specialty Contractors Registration & Listing
Zoning Clearance (includes $100 processing fee) (1 page)
Building Permit Application and Plan Examination - New Construction (4 pages) (use this application for all large residential and commercial projects)
Miscellaneous Permit Application - Residential Additions or Alterations (2 pages) (use this application for smaller projects such as the additon or repair of a deck, porch, shed, roof, fence, garage, or pool; or demolition)
Addition or Alteration Permit Application (use this application for ALL Commercial additions / alterations; use this application for any Residential project when more than one contractor is working on site)
Sign Permit Application (use this application for permanent and/or temporary signage. Include a Site Plan showing the location and size of signage)
Electrical Permit Application (use this application when you are just doing electrical work)
Site Review Application (must be received at least one week prior to the Site Review meeting) (Updated 06/22/09)
Site Plan - Example Plan and Layout (submit your Site Plan with your New Construction Permit Application)
Silt Fence Installation / Gravel Entrace Guidelines (this information should be included on your Site Plan)
